Transgender Breast Imaging Care

Session Description:There is increasing awareness of gender diversity and utilization of gender-affirming care. Breast radiologists need to be aware of unique breast imaging considerations for transgender and gender diverse patients. A better understanding of risk and cancer outcomes is necessary, both to promote evidence-based care and to advocate for equitable health care access. In this session, we will review basic terminology surrounding gender identity in the context of culturally inclusive care, discuss current understanding of breast cancer risk and screening recommendations in transgender individuals by the ACR Appropriateness Criteria, and consider imaging findings and management issues encountered in clinical scenarios.

Session Description: This session will review the current breast imaging evaluation and management for patients younger than 30 years old, pediatric patients, patients who are pregnant or lactating, and male patients. ACR Appropriateness Criteria will be emphasized. The presentations will also provide case examples illustrating the variety of benign and malignant pathologies in these patient populations.

  • Utilize appropriate breast imaging evaluation and management for patients who are pregnant or lactating.

Implant Complications and ALCL

Session Description: Breast implants commonly are placed for breast augmentation and reconstruction. Both breast injections and implants are seen frequently in everyday imaging practices. This case-based review will review implant imaging guidelines, implant types, direct injection appearances, implant rupture, antibiotic beads, benign and malignant tumors, complications such as hematomas, contracture, and infection. We will also cover confusing imaging findings specifically associated with breast implant surgery such as dermal matrix and expanders.

  • Review breast implant types, breast augmentation injection appearances, and imaging guidelines.
  • Illustrate imaging findings of common and uncommon complications associated with breast augmentation.
  • Identify imaging features of ALCL and other malignancies associated with breast augmentation.

The Reconstructed and Augmented Breast

Session Description:The purpose of this session is to review normal imaging findings after implant or autologous flap reconstruction. Common and less commonly encountered benign findings will be discussed in a case-based format. In addition, the rare complication of breast implant associated anaplastic lymphoma (BIA-ALCL) will be reviewed in detail including pathophysiology, imaging findings, screening guidance and management.

 

Learning Objectives: 

  • Upon completion, participant will be able to describe the imaging appearances of implant related lymphomas.
  • Recognize the normal multi-modality imaging appearance of a reconstructed breast including acellular dermal matrix (ADM), venous coupler device and also identify less common benign findings.
  • Discuss imaging appearance after implant and flap reconstruction and review findings associated with implant rupture and cancer recurrence.
