SBI Symposium Collection: Crack the Case: Breast MRI & CEM

Contrast enhanced mammography (CEM) and breast magnetic resonance imaging (MRI) are contrast-enhanced advanced breast imaging modalities that many radiologists find challenging to interpret. Indeed, surveyed attendees of the 2023 SBI Symposium request breast MRI and CEM as the top two breast imaging modalities for continuing education.  This highlights the need for additional educational activities to review the application and diagnostic challenges in the detection and characterization of breast malignancies at CEM and breast MRI. This collection features presentations from the 2023 SBI Symposium.

Course Objectives
  • Compare the performance of abbreviated breast MRI (AB-MRI) to full protocol breast MRI.
  • Apply the CEM BI-RADS lexicon in characterizing various CEM findings.
  • Identify the essential components of a contrast enhanced mammography (CEM) program.
  • Differentiate between breast cancer recurrence and benign post-operative change following breast reconstruction.
  • Identify and appropriately characterize breast malignancies, benign breast disease, and background parenchymal enhancement at breast MRI.
Crack the Case: Abbreviated and Full Protocol MRI Cases

Session Description: Experts in breast imaging will present interesting or tricky breast MRI cases to enhance participant interpretation skills. Abbreviated and Full Protocol examples will be covered in this session with emphasis on providing teaching points and practice pearls.

Learning Objectives: 

1. Illustrate common pitfalls in breast MRI interpretation.

2. Upon completion, participants will be able to recognize common patterns of BPE to minimize false-positive interpretation.

3. Upon completion, participants will be able to apply evidence-based recommendations and interpretation of breast MRI findings.

CEM: Doing It, Reading It, Reporting It

Session Description: This session will explore the steps to implement a successful contrast enhanced mammography program and discuss how to integrate this technology into the clinical care of our patients. Following a short didactic presentation, a series of cases will be presented to help attendees appreciate tips on successful implementation, apply the CEM BIRADS lexicon to image interpretation, and determine the next steps in patient management, including CEM-guided biopsy.

Learning Objectives:

1. Upon completion of this session, participants will be able to describe the steps in implementing a successful CEM program into a breast imaging practice.

2. Upon completion of this session, participants will be familiar with the CEM BIRADS lexicon and apply it to variety of cases.

3. Upon completion of this session, participants will be able to understand how to integrate CEM imaging findings into subsequent patient management.

Crack the Case: Challenging Multimodality Cases

Session Description: Join us for this case-based, interactive session! Review challenging cases, potential pitfalls (& how best to avoid them), and use of multiple breast imaging modalities and biopsy to arrive at a diagnosis efficiently. Discuss multiple possible approaches to a difficult case, various approaches to management recommendations, difficult rad-path concordance, and more!

Learning Objectives: 

  1. Recognize subtle nonmass findings at US, detect breast cancers on MBI, and direct axillary imaging for abnormalities identified on MRI.
  2. Compare and contrast cancer recurrence from benign post-op skin changes and fat necrosis following breast reconstruction.
  3. Correctly detect breast findings on incomplete non-contrast MRI exams and CT; identify and manage cases of multiple, bilateral architectural distortions.
